- Request form for the “trascrizione atto di nascita” to be completed and dated by the interested Italian citizen parent, and signed by both parents;
- Original birth certificate of the minor equipped with an “Apostille” (or legalization, for countries not adhering to the Hague Convention on the Apostille). United States certificates must always be in the extended format (so-called “Long Form” or “Extended Form” which reports the exact place of birth, date or age, and place of birth of the parents), according to the procedures by which such certificates are issued in individual States;
- Italian translation of the certificate, typewritten, complete, faithful to the original, with dates indicated in the day/month/year format. The translation can be done by the applicant; if done by a translator, it must be appropriately notarized and “apostillata” or legalized if the birth occurred outside the USA. A list of translators is available on the website of this Consulate;
- For children born out of wedlock, the recognition of paternity “Affidavit of Parentage” (or equivalent) must be presented, “apostillato” and translated using the same procedures as the other acts;
- Copy of valid passports of the parents (only the page with the photo, data, and signature) and of any foreign passport of the minor, if possessed;
- “Atto integrale di nascita” of the parent AND of the Italian grandparent issued by the Italian “Comune” of birth;
- “Certificato storico di residenza” of the grandparent, issued by the competent Italian “Comune” (last Municipality of residence in Italy or “AIRE” registration); if periods of residence abroad appear on the historical certificate, it is necessary to attach documentation certifying the non-naturalization of the grandparent at the time of the grandchild’s birth in the foreign countries of residence. If the spouse of the grandparent is of a different citizenship, it must be proven that the grandparent never acquired that citizenship through marriage. Please note: sworn statements (“affidavit”) will not be accepted; only documentation issued by official sources, duly legalized and translated, will be accepted. By way of example, the following may be produced: negative certificates of citizenship, attestations of renunciation of citizenship; a copy of a valid visa issued by the competent United States authorities; or a copy authenticated by a “notary public” of the certificate of naturalization issued by the competent foreign emigration authorities or of a valid United States permanent resident card (“Green card”);
- Pre-stamped and self-addressed USPS envelope for the eventual return of incomplete/incorrect documentation.
