    • Request form for the “trascrizione atto di nascita” to be completed and dated by the interested Italian citizen parent, and signed by both parents;
    • Original birth certificate of the minor equipped with an “Apostille” (or legalization, for countries not adhering to the Hague Convention on the Apostille). The translation can be done by the applicant; if done by a translator, it must be appropriately notarized and “apostillata” (if done by a translator) or legalized if the birth occurred outside the USA. A list of translators is available on the website of this Consulate;
    • For children born out of wedlock, the recognition of paternity “Affidavit of Parentage” (or equivalent) must be presented, “apostillato” and translated using the same procedures as the other acts;
    • Copy of valid passports of the parents (only the page with the photo, data, and signature) and of any foreign passport of the minor, if possessed;
    • Certificate of citizenship of the Italian parent indicating the date of acquisition of Italian citizenship, issued by the competent Italian “Comune” (last Municipality of residence in Italy or “AIRE” registration);
    • “Certificato storico di residenza” issued by one of the Italian “Comuni” where the parent resided, showing that the parent resided in Italy for two consecutive years before the birth of the minor and after the acquisition of citizenship;
    • Pre-stamped and self-addressed USPS envelope for the eventual return of incomplete/incorrect documentation.

    N.B.: The Italian citizen applicant’s residence in Italy prior to the acquisition of Italian citizenship is not relevant.
