The form must be dated and signed in front of a Visa Officer.<\/li>\n<li><strong>Recent passport-size photograph<\/strong>, on white background, full face and front view, stapled\/glued to the application form. Scanned or photocopied photos will NOT be accepted;<\/li>\n<li><strong>Valid passport plus 1 copy<\/strong>\u00a0of the page with photograph and expiration date. The passport or travel document must be valid at least three months after the visa expiration date. Please make sure the passport has at least two blank pages to affix the visa;<\/li>\n<li><strong>For Non-US citizens: original and copy of Permanent Residence Card<\/strong>\u00a0OR other US immigration document allowing re-entry into the United States (e.g., valid long-term US visa, Advanced Parole, etc.). International students must submit original and copy of F-1 Visa and\u00a0valid re-entry(\u201ctravel\u201d) signature on\u00a0I-20;<\/li>\n<li><strong>Copy of Driver\u2019s License or State ID<\/strong>\u00a0as proof of residence in the jurisdiction of this Consulate General. Full-time students may also use original and copy of Student ID. If you recently moved and have not updated your ID, you must submit another proof of current address (e.g., utility bills, bank statements, etc.);<\/li>\n<li><strong>Copy of the letter addressed by the Italian Sport Organization<\/strong>\u00a0to the Italian Police Authorities, the Sport Federation and to the applicant, requesting the permit to engage the athlete, specifying the type of sport activity and the duration of the contract;<\/li>\n<li><strong>Letter of assent from CONI<\/strong>\u00a0mailed directly to the Consulate from CONI itself;<\/li>\n<li><strong>Confirmed flight itinerary;<\/strong><\/li>\n<li><strong>Visa fee<\/strong>\u00a0<strong>(under Consular Fees on home page)<\/strong>: (non refundable) in the form of Money Order or cashier\u2019s check, payable to the Consulate General of Italy in Boston, with applicant\u2019s name and address as the remitter;<\/li>\n<\/ul>\n<p><strong>Please note:<\/strong>\u00a0This Consulate General\u00a0reserves the right to request further documentation.<\/p>\n","protected":false},"excerpt":{"rendered":"Documents check-list Visa Application Form (up to 90 days &#8211; Over 90 days)\u00a0Please notice that all the names appearing on the passport must\u00a0 appear on the application form (all data must match those on the passport).
