{"id":795,"date":"2023-05-25T10:00:08","date_gmt":"2023-05-25T14:00:08","guid":{"rendered":"https:\/\/consboston.esteri.it\/servizi-consolari-e-visti\/servizi-per-il-cittadino-italiano\/rimpatrio-salme-o-ceneri\/"},"modified":"2023-10-26T12:38:27","modified_gmt":"2023-10-26T16:38:27","slug":"rimpatrio-salme-o-ceneri","status":"publish","type":"page","link":"https:\/\/consboston.esteri.it\/en\/servizi-consolari-e-visti\/servizi-per-il-cittadino-italiano\/rimpatrio-salme-o-ceneri\/","title":{"rendered":"Repatriation of remains or ashes"},"content":{"rendered":"<p>This Consulate General has jurisdiction over the following states: Maine, Massachusetts, New Hampshire, Rhode Island, Vermont.<\/p>\n<p>The amount of consular perceptions due, payable only by money order or cashier&#8217;s check, will be determined when complete documentation is delivered to the Consular Office.<\/p>\n<p>&nbsp;<\/p>\n<p>The following documentation is required for the <strong>repatriation of a body<\/strong> to Italy:<\/p>\n<ul>\n<li>Letter from the family members of the deceased in which they authorize the Funeral Home (funeral home firm) to repatriate the body to Italy;<\/li>\n<li>Statement from the Funeral Director requesting the Consulate to proceed with the repatriation of the body to Italy; the Funeral Director&#8217;s signature must be legalized by a Notary Public;<\/li>\n<li>Certified full copy (&#8220;certified full copy&#8221;) of the death certificate, stating the cause of death; the death certificate must have an Apostille and an Italian translation, also with Apostille; for married women, the maiden name must be legible;<\/li>\n<li>Copy of Italian passport and &#8220;permanent resident card &#8211; green card&#8221; (in case of exclusively Italian citizen); if U.S. citizen, copy of naturalization certificate and copy of old Italian passport (or birth certificate);<\/li>\n<li>Certificate from the local Department of Health stating the absence of infectious diseases in the area of death, with Apostille;<\/li>\n<li>Notarized statement bearing the Apostille of the Funeral director as per the <a href=\"https:\/\/consboston.esteri.it\/wp-content\/uploads\/2023\/05\/affidavitsalme.pdf\" target=\"_blank\" rel=\"noopener\">attached form<\/a>;<\/li>\n<li>Information sheet from the Funeral Home regarding the deceased that shows the following information:\n<ul>\n<li>Full name (maiden name for married women);<\/li>\n<li>Place and date of birth (including province);<\/li>\n<li>Airport of departure from the United States;<\/li>\n<li>Airport of arrival in Italy;<\/li>\n<li>Municipality and province of burial in Italy;<\/li>\n<li>Name, address and telephone number of the funeral home that will receive the body in Italy.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>The following documentation is required for the <strong>repatriation of ashes<\/strong> to Italy:<\/p>\n<ul>\n<li>Letter from the family members of the deceased in which they authorize the Funeral Home (funeral home company) to repatriate the ashes to Italy;<\/li>\n<li>Statement from the Funeral Director requesting the Consulate to proceed with the repatriation of the ashes to Italy;<\/li>\n<li>Certified full copy (&#8220;certified full copy&#8221;) of the death certificate, stating the cause of death;
