United States certificates must always be in the extended format (so-called &#8220;Long Form&#8221; or &#8220;Extended Form&#8221; which reports the exact place of birth, date or age, and place of birth of the parents), according to the procedures by which such certificates are issued in individual States;<\/li>\n<li>Italian translation of the certificate, typewritten, complete, faithful to the original, with dates indicated in the day\/month\/year format. The translation can be done by the applicant; if done by a translator, it must be appropriately notarized and &#8220;apostillata&#8221; (if done by a translator) or legalized if the birth occurred outside the USA. A list of translators is available on the website of this Consulate;<\/li>\n<li>For children born out of wedlock, the recognition of paternity &#8220;Affidavit of Parentage&#8221; (or equivalent) must be presented, &#8220;apostillato&#8221; and translated using the same procedures as the other acts;<\/li>\n<li>Copy of valid passports of the parents (only the page with the photo, data, and signature) and of any foreign passport of the minor, if possessed;<\/li>\n<li>Certificate of citizenship of the Italian parent indicating the date of acquisition of Italian citizenship, issued by the competent Italian &#8220;Comune&#8221; (last Municipality of residence in Italy or &#8220;AIRE&#8221; registration);<\/li>\n<li>&#8220;Certificato storico di residenza&#8221; issued by one of the Italian &#8220;Comuni&#8221; where the parent resided, showing that the parent resided in Italy for two consecutive years before the birth of the minor and after the acquisition of citizenship;<\/li>\n<li>Pre-stamped and self-addressed USPS envelope for the eventual return of incomplete\/incorrect documentation.<\/li>\n<\/ul>\n<p><strong>N.B.: The Italian citizen applicant&#8217;s residence in Italy prior to the acquisition of Italian citizenship is not relevant.<\/strong><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"(self-declarations are not accepted) Request form for the &#8220;trascrizione atto di nascita&#8221; to be completed and dated by the interested Italian citizen parent, and signed by both parents; Original birth certificate of the minor equipped with an &#8220;Apostille&#8221; (or legalization, for countries not adhering to the Hague Convention on the Apostille).
