Shipment of human ashes to Italy

This Consulate General is competent for the States of Maine, Massachusetts, New Hampshire, Rhode Island, Vermont.

  • A letter from the family of the deceased, authorizing the funeral director to ship the ashes to Italy;
  • Notarized affidavit from the funeral home director requesting the Consulate to transfer the ashes of the deceased to Italy for burial;
  • 1 certified full copy of the death certificate, stating the cause of death, complete with an Apostille as well as a notarized Italian translation also with an Apostille; for married women: maiden name must appear;
  • 1 cremation certificate complete with an Apostille as well as a notarized Italian translation also with an Apostille;
  • 1 notarized affidavit with an Apostille from funeral home director as per attached form;
  • 1 data sheet for the deceased on funeral home letterhead stating the following information:
    • exact name and surname (maiden name for women);
    • exact date and place of birth, including name of city/town and province;
    • airport of departure from the US; airport of arrival in Italy;
    • exact city / town and province of burial in Italy;
    • address and phone number of receiving funeral home;
  • Copy of Italian passport and green card (if only Italian citizen), naturalization certificate (if naturalized US citizen), certificate of birth;

The consular fee, payable by money order or cashier check will be determined when the complete paperwork is ready to be submitted to the Consulate.
